Family Nurse Practitioner

Job Title: Family Nurse Practitioner
Location: Sayre Health Center – Philadelphia, PA 19139
Job Type: Full-Time; 4-day on-site schedule

Overview:
Sayre Health Center (SHC), a mission-driven Federally Qualified Health Center in West Philadelphia, is seeking a full-time Family Nurse Practitioner to provide high-quality care to underserved communities. Join a collaborative care team and help make a lasting impact. This role offers a predictable weekday off while preserving continuity, extended hours coverage, and comprehensive primary care.

Responsibilities:
- Provide patient-centered primary care across the lifespan, including acute, chronic, and preventative care.

- Complete histories, physical examinations, health screenings, diagnoses, and individualized treatment plans.

- Order and interpret laboratory tests, diagnostic studies, and imaging; communicate clinically significant results and close follow-up loops.

- Prescribe and manage medications, vaccinations, and appropriate therapies; educate patients about diagnoses, prevention, and practical barriers to treatment.

-Address diabetes, hypertension, preventative screening, immunization, and other quality-care gaps during appropriate encounters.

-Coordinate care with physicians, nurses, CMAs, behavioral health clinicians, outreach staff, pharmacists, specialists, and community partners.

-Complete encounter documentation within 24 hours and manage assigned results, messages, refills, referrals, and prior-authorizations within established timeframes.

-Participate in quality improvement, clinical education, team meetings, occasional pre-scheduled outreach events, and teaching when assigned.

Productivity & Quality Expectations:

This position uses a transparent productivity model intended to support access without compromising care quality. After a 90-day ramp-up period, the initial expectations is an average of 16 completed patient visits per full clinical day. Targets are reviewed with the provider and adjusted for visit length, clinical acuity, new-patient mix, procedures, approved administrative duties, and assigned teaching or outreach responsibilities.

- Provider performance includes completed visits, timely chart closure, results and inbox management, continuity, quality measures, patient access, and professional teamwork.

-Patient cancellations and no-shows are not treated as provider underperformance when the provider was available and completed assigned clinical work.

- Productivity and quality reports are reviewed regularly so barriers can be corrected early; performance above target may qualify for an incentive under Sayre Health Center's approved bonus plan.

Required Qualifications:

- Master's or doctoral degree from an accredited Family Nurse Practitioner program.

- Current Pennsylvania Certified Registered Nurse Practitioner license and national FNP board certification, or eligibility to complete both before starting.

- At least one year of post-graduate clinical experience in family medicine, internal medicine, primary care, urgent care, or a comparable outpatient setting.

-Pennsylvania prescriptive authority, DEA registration, and current BLS certification, or eligibility to complete before independent practice.

-Ability to use an EHR reliably, complete documentation and clinical follow-up on time.

-Strong communication, teamwork, cultural humility, and commitment to caring for underserved communities.

Preferred Qualifications:

- Experience in an FQHC, community health center, or other safety-net setting.

-Experience with pediatric care, women's health, cervical screening, or chronic-disease management.

- Experience with eClinicalWorks or comparable EHR systems.

- Spanish or another language commonly spoken by the community is a plus.

Schedule:

- 4-days on-site work week

- 1 scheduled work-week day off

-Exact days and hours will be established

- Weekend community events as scheduled

Compensation & Benefits:
- Salary: $120,000 – $130,000 (commensurate with experience, credentials, and approved schedule)
